ATHLETE REGISTRATION FAQ's

Who is required to get a NEW physical?
- All current Freshman, Juniors, and transfers are required to get a physical exam for the 23-24 school year.
My athlete is a sophomore/senior, do they need a physical?
- No, however, you do need to prove that a physical was done last year. On Aktivate, you will upload the old physical to the Physical Exam and Consent Form section. ALL athletes are required to have a physical uploaded.
I can't find my athlete's physical from last year. What do I do?
- SportsWare(SWOL) was our old system last year, and is expired. Access is not available to any Rocky parent, player or staff member. Therefore, a physical cannot be retrieved from the site. Check your files at home to see if you stored it somewhere. Next, try searching your phones/laptops/computers, for a picture or downloaded image of it. You had to upload it last year to our old system, so it was downloaded somewhere. You can try calling the clinic your athlete got the physical from and ask to pick up a copy (if done at Rocky, we do NOT have a copy of it, as you took it home with you). If all options fail, a new physical exam will need to be done and uploaded onto Aktivate before the start of tryouts/practice.
My athlete played a fall or winter sport this year, do I have to do anything for the spring semester?
- Log onto Aktivate and check your athletes' registrations. If they are registered for their perspective spring sport and had all the requirements in for their fall/winter sport, then they are good to go. If the spring sport is not showing up on the registration, edit the registration and add the spring sport.
What is the allergy and anaphylaxis emergency plan form and baseline concussion test?
- The allergy form is great information for us to have if your athlete has a known, severe allergy. Please fill in the form and upload here if that is the case. The form is OPTIONAL, however, it may show as incomplete on your end. This is ok, your athlete is still cleared to play if nothing is uploaded here. If you don't like seeing it as "missing", you can upload a blank form or anything to show as completed. The baseline concussion testing may or may not be completed with the athletic trainer at the start of the season, so you may ignore this.
When is Aktivate registration and physical exam due?
- Before day 1 of tryouts/practices. Your athlete will be checked during the start of practices and will be asked to sit out or leave until Aktivate is complete.

West Ada is a pay-to-participate district, and all RMHS participants are guaranteed a spot on a team.

Players must purchase an activity card and pay a district participation fee, before Friday, March 1st, 2024.
