Click the following link to fill out the Participation Form online through Aktivate - you will need to register for Aktivate in order to complete the required paperwork.

Prior to the starting date of EACH SPORT, athletes must:

• Online Forms Filled out: Participation Forms (Aktivate)

  • Must be marked for each individual sport.

  • Aktivate Parent Instructions

• Pay the $22.25 LHMS activity card fee. (One-time fee).

• Pay the district required $90 athletic fee. (Per Sport) to Lake Hazel Middle School. (If your child participates in more than two sports, you only need to pay the $90 fee twice, the rest of the sports will be free).

• Pay both payments to the Lake Hazel bookkeeping office prior to the season start date.
