6th grade intramural participation form

Concussion Form

6th graders may participate in our 6th grade intramural volleyball program. These are scheduled on dates when our 7th and 8th graders will be playing away matches so that we have gym space for intramurals.

You will not use Aktivate to sign up for intramural programs. Please fill out the form to the left and turn it in to the main office prior to the start date.

It is a one-time fee Activity Fee of $22.25. You only pay this one time for the entire year. You can attach a check to the form when you turn it in, or you can pay online by clicking the "My School Fees" button on our website.

6th graders may also participate in the following school athletic programs:

-Cross Country or Cheer in the fall

-Wrestling in October and November

-Spring Track

-Spring Tennis
