Application Process:

1. Download the application by clicking the link below.

2. Fill out the application. (List the grade your student will be in for the 2023-2024 school year in the grade applying section). ** If CMS is full for any grade for the 2023-2024 school year, all new applications will be immediately placed on the waiting list. Wait list students will be contacted for interviews prior to considering enrollment. CROSSROADS ALWAYS ACCEPTS APPLICATIONS!

3. Submit the application by emailing it to jensen.rikki@westada.org

4. A Crossroads staff member will contact you upon receipt of your student's application. All students and a parent/guardian must attend an open house OR a personal interview prior to consideration of enrollment.

    Notification of acceptance, waitlist, or denial will be announced within two weeks of interview.
